Homelessness Prevention

  • Housing relocation & stabilization services

  • Short-term rental assistance to avoid placement in emergency shelter

  • Help to regain stability in current housing or move into other permanent housing

Street Outreach

  • Connecting unsheltered homeless individuals & families with emergency shelter

  • Providing homeless individuals or families with critical health & mental health services

  • Arranging urgent, non-facility-based care

Rapid Rehousing

  • For those that do not need intensive or ongoing support to exit homelessness & return to permanent housing

  • Quickly return individuals & families to permanent housing

  • Assistance to identify & build strengths to maintain their own housing
